Currently if I am uncertain about an edit I make, I do the following:
Check the “review requested” in JOSM.
Describe in the description what I am uncertain about.
However, the second seems like it is the wrong place.
The description should be about what was changed and why.
Is there a seperate tag to describe why I requested a review?
Something like description=Fixed routes that are not part of a route_master review_requested=yes review_reason=I am somewhat new to editing PTv2 and am worried I might have accidentally broken something
I do not see an issue here. The comment is intended to be read by other mappers and not for machine evaluation. I do not consider further structuring to be necessary.
comment=* – describing why a mapper made that group of changes, or what was changed.

While optional, mappers are encouraged to make full use of this tag by setting a meaningful, human-generated description (not an automated message), as it will show up almost everywhere where the changeset is listed, and is likely to be read by fellow mappers to try and understand what has happened.
