this is about parsing existing ones, not about adding new ones (unless OM users are not adding any OH with this dashes?)

I know, but the problem exists on both ends: editors adding it, and apps not properly parsing them (such as the parser used by StreetComplete).

Are there any other editing maps where users added them?

I don’t know, I didn’t do a full analysis on that. On a quick manual inspection, they come mainly from iD, but per my last message, JOSM also doesn’t flag nor fix it immediately.
